Do you live in Bletchley? We need your help!

We're supporting the Milton Keynes Health and Care Partnership with a project to develop services in local neighbourhoods across Milton Keynes that help us to live a longer, healthier life.

We hear from people every day who are experiencing ill health. When you're unwell, it affects every aspect of your life, from finances to your relationships. 

That's why we're helping our local NHS, Milton Keynes City Council, and other organisations (together they're called the Milton Keynes Health and Care Partnership) with a project to find out how we can plan community support, and health and care services, that keep us in the best possible health, for the longest possible time. 

What would encourage you to take up healthy habits, but also get support from healthcare professionals when you need it, so that you can stay on top of your health, and keep doing the things you love? 

The pilot project in Bletchley.

The MK Health and Care Partnership are trialling a new way of working in Bletchley, through a project called the 'Bletchley Pathfinder'. The project hopes to:

  • Help people to stay healthier as they grow up and age.
  • Make sure that people living with more than one long term health condition and complex health and social care needs get support that works for them.
  • Offer better support to children and young people with their mental health.

To make this a success, they'd like to understand the things that might encourage you to take up healthy habits such as regular exercise, quitting smoking or cooking healthy meals. What support services would help you to manage your mental health? Which health and care services are absolutely essential to Bletchley residents? 

What changes are being considered? 

The main aim is to design health and care services in Bletchley that are:

  • Proactive and personalised: residents get the support, resources and encouragement they need to stay well.
  • Delivered by joined up, connected teams of health and care professionals close to where people live.
  • Geared up to support residents who receive care for multiple conditions such as diabetes, asthma, high blood pressure; and those who use adult social care services.
