Integrated Discharge Hub unveiled after our research into experiences of hospital discharge 

A new team at Milton Keynes University Hospital that combines professionals across health and social care, will now give residents increased support after a stay in hospital.

This year, our team spent some time listening to patients' experiences of hospital discharge at Milton Keynes University Hospital. The BLMK ICB asked us to explore how the process is working for people, and make recommendations on how it could be better.

We shared our findings with them in the Autumn, and this week a new service known as the MK Integrated Discharge Hub has been launched. The Hub is centred around a diverse team of specialists from across health and social care who are on hand when a patient is discharged, to give them increased support after a stay in hospital.

The initiative has been set up by Milton Keynes University Hospital, Milton Keynes City Council, and Central and North West London (CNWL) NHS Foundation Trust.

By combining services, the hub will act as a central point of contact which simplifies the process, reduces duplication, and minimises the likelihood of delays caused by moving between services – a common issue across the country.
